What is Subtotal / Near-Total Thyroidectomy?
Subtotal or near-total thyroidectomy is the surgical removal of the majority of the thyroid gland while leaving a small remnant (1–4 grams) on one or both sides, typically near the recurrent laryngeal nerve and parathyroid glands. Once the standard treatment for Graves' disease and large multinodular goitre, it is now used selectively — primarily when total thyroidectomy carries increased risk to nerves or parathyroids. How the Procedure Works
Anaesthesia & Positioning
General anaesthesia is given; the patient is positioned supine with the neck extended.
Collar Incision
A 5–6 cm transverse incision is made in a natural skin crease for optimal cosmetic outcome.
Bilateral Mobilisation
Both lobes are mobilised; superior and inferior thyroid vessels are controlled.
Remnant Preservation
A small posterior remnant is left near the recurrent laryngeal nerve entry and parathyroid glands.
Resection & Closure
The bulk of the thyroid is removed; haemostasis is confirmed; strap muscles and skin are closed.
